Who's an ideal candidate for rhinoplasty? Actually anyone who wants an improvement of the cosmetic appearance of their nose. In my thirty year history of doing rhinoplasty surgery we have people who want a little tiny tweak to the nasal tip and we have people that we need complete nasal reconstruction or more difficultly which is probably the most difficult thing in cosmetic or plastic surgery is someone who's had a previous nose job or rhinoplasty and even more difficult has had several of them. So when we look at the nose from an aesthetic standpoint, it's what you as the patient wants. Okay?
Dr Robert J Troell M.D:Well, I'll give my recommendation what I think aesthetically is the best. Often as an adjunctive procedure to rhinoplasty, we do chin surgery. If you have a retruded chin, micronathia it's called, if your chin is too small, if we put a chin implant at the same time we do a rhinoplasty, a significant impact on your result. So consideration of the whole face, we want it to be harmonious, with your face, the anesthetic result. So what can we do to make your the ideal result being the ideal candidate?
Dr Robert J Troell M.D:Well, we listen to what you have to say. Do you want tip work done? If you do, most people have a certain position of the tip that they want. Do they want a certain rotation? Do they want narrowing?
Dr Robert J Troell M.D:Do they want, a little bit of super tip depression? Do they want their bridge taken down? Do they want it narrowed? So all those considerations are performed during your aesthetic consultation. So everybody who wants a rhinoplasty that has a concern on their aesthetic, appearance is an excellent candidate.
Dr Robert J Troell M.D:In terms of what we do is based on your feedback with me during the consultation. So what we want to get the best result without a revision rate in general, the American Academy of Facial Plastic and Reconstructive Surgery shows the revision rate is about eight to ten percent in The United States. Our goal is to do the procedure so you don't need a second procedure to get the results you're looking for. So the most important thing is getting good structural support to your nose, that your nose is stable, that's not gonna droop or push to one side or the other. So cartilage grafts are important in that regard.
Dr Robert J Troell M.D:We either take it from the nasal septum or the ear. And then for the aesthetic appearance to your nose, get to get the ideal result, sometimes we will morph your nose. What does that mean? We take a photograph of you from a side view or a bleak view. We then change it with a software program so you yourself can see what the end result will be.
Dr Robert J Troell M.D:So we use your input after that morphing that photograph so we know how much to take the the bridge down. So morphing also helps determine if you need a chin implant. If you have augmentation of your chin, often it makes someone who has a large nose significantly smaller. So photomorphing can help you see what your nose is gonna look like so we can get the ideal rhinoplasty that you're looking for.
